First Visit

Your consultation appointment will give Dr. Thomas the opportunity to review your medical history and perform and oral and radiographic exam to properly diagnose your condition.  A treatment plan will then be tailored to your individual needs. At this appointment you should ask any questions that you may have.

Please arrive early for your consultation appointment to complete any paperwork and check in. A parent or legal guardian must be present if a patient is under the age of 18.

Please remember to bring the following with you to your first visit:

• Referral slip 

• Health and/or dental insurance cards, if applicable 

• Driver’s license

• List of all current medications.  For your health and safety, it is important to alert us to all current and prior medical conditions you may have.
